What is the primary goal of this questionnaire?
To determine the numbers of days you were on leave for each valid period (there are 2) and for each valid situation (there are 3). This will be key information to calculate your total Claim.
Which dates are included in each qualified period?
Qualified periods are only 2 and are treated completely separated:
1st Period: from January 1, 2021 to March 31, 2021 = (Q1 2021).
2nd Period: from April 1, 2021, to September 30, 2021 = (Q2 & Q3 2021).
What is the maximum amount of days I can claim per period and per situation?
1st Period: (01/01/21 to 03/31/21) > 10 Days + 50 Days = 60 Days maximum.
2nd Period: (04/01/21 to 09/30/21) > 10 Days + 60 Days = 70 Days maximum.
What are the qualified Covid-19 related situations that could have affected me?
There are 3 possible situations considered:
a) If... you were out on leave because you had Covid or had symptoms or a doctor ordered you to quarantine or you were subject to a federal/state/local quarantine or isolation order.
b) If... you were out on leave because you were caring for a household member that had Covid or had symptoms or a doctor ordered to quarantine or caring for an individual who was subject to a federal/state/local quarantine or isolation order.
c) If... you were caring for a son or daughter (under 18yo) because the school or place of care for that child was closed or the childcare provider for that child was unable due to COVID-19 precautions or for an individual of your household who was physically or mentally limited to self-care.
What is the break-out of qualified number of days of leave I can claim in relation to different situations?
Period 1:
a) Situation 1 (self-care) = max. 10 days.
b) Situation 2 (care for others) = max. 10 days.
c) Situation 3 (care for child under 18yo or disabled individual) = not needed (max. 50 days).
Period 2:
a) Situation 1 (self-care) = max. 10 days.
b) Situation 2 (care for others) = max. 10 days.
c) Situation 3 (care for child under 18yo or disabled individual) = not needed (max. 60 days).
What dates do I need to remember exactly?
If the dates you are trying to input on the different situations overlap with another situation that occurred at the same time-period, then please don’t include that date twice. Dates from one situation cannot overlap with other situations' dates.
Period 1:

b) Situation 2 (care for others) = exact dates (max. 10)
c) Situation 3 (care for child under 18yo or disabled individual) = not needed (max. 50)
Period 2:

What documentation is required?
You need to have your complete tax returns (1040 + all schedules, especially the “SE”) in hand to answer this questionnaire. We will be asking about your Self-employed situation in 2020 and 2021, the company type, the EIN, and other specific information that you will be easily able to find in your Tax Returns and Schedules.
2020: Form 1040 + schedules, and W-2
2021: Form 1040 + schedules, and W-2
What other information is required?
If you received W-2 wages during these mentioned periods, it is extremely important that you let us know prior to starting this questionnaire. So, remember, if you received “Sick and Family Leave wages” contact us, because these amounts need to be discounted from your potential claim, and we will need to determine the exact amount you received to be able to determine your available FFCRA Tax Credit. If these cases, additional information might be required from you.
Other considerations:
Regarding dates:
a) While completing this questionnaire, please always indicate the dates in the following format: “MM/DD”. As the year will always be 2021, please do not include it. Always use 2 digits for “MM = Month” and for “DD = day”.
b) Separate each date by a comma and a space following it (“, ”).
c) This is an example on how to input all dates: “02/18, 02/19, 02/22, 02/23”. Notice that you have to take weekends into account (02/20 and 02/21) and not include those dates in your Claim if not corresponding.
